ABSTRACT

Cow dung powder coloring agent poisoning is common in Southern Tamil Nadu. Both yellow and green varieties are common. Yellow cow dung poisoning usually produces central nervous system (CNS) and hepatic involvement as well as gastrointestinal problems. Though cardiac issues like arrhythmias are seen, toxic myocarditis and cardiac failure are not common. We present a case of a 42-year-old lady with yellow cow dung poisoning who developed toxic myocarditis and cardiac failure with complete recovery over a period of time.

ABSTRACT

We aimed to carry out a systematic review and meta-analysis of the effects of bariatric surgery on fertility in men with obesity. To assess the methodological quality of the included studies, the Cochrane-ROBINS-I tool for non-randomized studies was used. Three hundred and eighty-four articles were found. A statistically significant improvement in total testosterone levels was evidenced in all the patients; some studies did not find a correlation between this increase and the improvement in seminal parameters. Only one article showed a slight increase in estradiol levels (not statistically significant, 33.3-32 pg/mL, p: 0.68). Normal or low levels of LH and FSH were present both before and after the procedure. The total sperm count decreased at 6 months and was significantly lower at 12 months. There was not evidence of changes in seminal volume, sperm motility, and vitality. Although our systematic review shows changes on sexual hormonal parameters in men, with an increase in total testosterone levels, a negative one was also found on semen quality. More prospective and randomized studies are required, hopefully of the Latin American population, which can help confirm this association


Nuestro objetivo fue realizar una revisión sistemática y un metaanálisis de los efectos de la cirugía bariátrica sobre la fertilidad en hombres con obesidad. Para evaluar la calidad metodológica de los estudios incluidos, se utilizó la herramienta Cochrane-ROBINS-I para estudios no aleatorios. Se encontraron 384 artículos. Se evidenció una mejoría estadísticamente significativa en los niveles de testosterona total en todos los pacientes; algunos estudios no encontraron una correlación entre este aumento y cambios en los parámetros seminales. Solo un artículo mostró un ligero aumento en los niveles de estradiol (no estadísticamente significativo, 33,3 a 32 pg/ml, p: 0,68). Los niveles normales o bajos de LH y FSH estaban presentes tanto antes como después del procedimiento. El recuento total de espermatozoides disminuyó a los 6 meses y fue significativamente menor a los 12 meses. No hubo evidencia de cambios en el volumen seminal, la motilidad y la vitalidad de los espermatozoides. Aunque nuestra revisión sistemática muestra cambios sobre los parámetros hormonales sexuales en hombres, con un aumento de los niveles de testosterona total, también se encontró uno negativo sobre la calidad seminal. Se requieren más estudios prospectivos aleatorizados, ojalá en población latinoamericana, que ayuden a confirmar esta asociación.

ABSTRACT

Objective@#We performed a meta-analysis of epidemiological results for the association between occupational exposure as a firefighter and cancer as part of the broader evidence synthesis work of the IARC Monographs program. @*Methods@#A systematic literature search was conducted to identify cohort studies of firefighters followed for cancer incidence and mortality. Studies were evaluated for the influence of key biases on results. Random-effects meta-analysis models were used to estimate the association between ever-employment and duration of employment as a firefighter and risk of 12 selected cancers. The impact of bias was explored in sensitivity analyses. @*Results@#Among the 16 included cancer incidence studies, the estimated meta-rate ratio, 95% confidence interval (CI), and heterogeneity statistic (I2) for ever-employment as a career firefighter compared mostly to general populations were 1.58 (1.14–2.20, 8%) for mesothelioma, 1.16 (1.08–1.26, 0%) for bladder cancer, 1.21 (1.12–1.32, 81%) for prostate cancer, 1.37 (1.03–1.82, 56%) for testicular cancer, 1.19 (1.07–1.32, 37%) for colon cancer, 1.36 (1.15–1.62, 83%) for melanoma, 1.12 (1.01–1.25, 0%) for non-Hodgkin lymphoma, 1.28 (1.02–1.61, 40%) for thyroid cancer, and 1.09 (0.92–1.29, 55%) for kidney cancer. Ever-employment as a firefighter was not positively associated with lung, nervous system, or stomach cancer. Results for mesothelioma and bladder cancer exhibited low heterogeneity and were largely robust across sensitivity analyses. @*Conclusions@#There is epidemiological evidence to support a causal relationship between occupational exposure as a firefighter and certain cancers. Challenges persist in the body of evidence related to the quality of exposure assessment, confounding, and medical surveillance bias.

This investigation presents the food and feeding activity of and endangered riverine catfish Rita rita, during February 2017-January 2018. A total of 225 fish individuals was analyzed for stomach contents by characterizing the dominant food items and morphometric features. The results divulged ten major food items consumed, preferably fish scales and eggs, teleost fishes, copepods, cladocerans, rotifers, and mollusks. Total length and body weight of fish varied between 9-34 cm (20.53 ± 6.90 cm) and 10-400 g (9125.94 ± 102.07 g), respectively. The index of relative importance (IRI%) showed the importance of rotifers over the other food items. Morisita's index of diet overlap indicated seasonal variations in catfish diets with summer and monsoon displaying the least overlap, while maximum overlap during monsoon and winter seasons. The non-metric multidimensional scaling (nMDS) indicated the close association between the food items available during summer and winter seasons with a significant difference among the seasons (ANOSIM, R = 0.638, P = 0.013). Levin's niche breadth index arranged in the order of 0.88>0.81>0.78>0.63>0.43 for the size classes of V, IV, III, II and I, respectively. The PCA explained 95.39% of the total variance among the food items and fish size groups. Small-sized fish individuals displayed a greater correlation with food items suitable for their mouth size. In conclusion, the variety and frequency of food items recorded indicated considerable feeding plasticity and opportunistic feeding behavior with a shift from carnivorous to omnivorous feeding nature. This study could render useful information on the food and feeding habits of R. rita and provide background for preparing its diet for future aquaculture practices.

ABSTRACT

The coronavirus disease 2019 (COVID-19) pandemic has raised alarm all over the world, whereby the mental health crisis is on the rise and people are in quest for resolution. So, the present review paper is an attempt to evaluate the previous work on mindfulness, and, its impact during the COVID-19 pandemic. Mindfulness as a multidimensional construct teaches us to regulate our awareness of the present moment followed by an attitude of non-judgemental acceptance. This paradigm shift in the field of psychology has created wonders in psychotherapeutic practice. The current review aims to highlight the ‘why’ and ‘how’ of mindfulness as intervention that could reach every person, be it the COVID-19 sufferers or the COVID-19 warriors. It also provides an insight into the efficacy of mindfulness as an intervention itself or in adjunct to other psychotherapeutic treatments. Therefore, it calls forth the need to put mindfulness-based interventions during this pandemic situation.

ABSTRACT

Background: Early adverse stress is a risk factor for the appearance of mental health ailments during adulthood. Aim: To systematically review treatment outcomes on mental health symptoms and functional domains, and of interventions aimed at treating adults with depressive disorders and early adverse stress (EAS). Material and Methods: Systematic review and meta-analysis including experimental and quasi-experimental published studies indexed in the CINAHL, EMBASE, PubMed, and Web of Science databases, which explored the effectiveness of treatment interventions for depressive disorders in adults exposed to EAS. Data on outcomes was extracted from the included studies. A narrative, qualitative approach or a quality-effects model for meta-analysis were used for synthesizing these data. Results: Thirteen studies were included. Psychological or combined treatment interventions for depression in adults exposed to EAS may be effective in reducing trauma-related symptoms and social dysfunction in the short-and mid-term, with small effect size and without substantive heterogeneity. The assessment of anxiety symptoms and health-related quality of life yielded mixed results. Conclusions: Despite the ubiquity of EAS and its adverse and long-lasting consequences for well-being and health, treatment alternatives are scant. This review suggests that there are treatment interventions for depression in adults exposed to EAS that may achieve integral mental health benefits, alleviating its impact on various symptoms and functional domains, when EAS is explicitly considered in the treatment intervention.

ABSTRACT

Arterial and venous atherothrombotic events are finely regulated processes involving a complex interplay between vulnerable blood, vulnerable vessel, and blood stasis. Vulnerable blood (‘thrombogenicity’) comprises complex interactions between cellular components and plasma factors (inflammatory, procoagulant, anticoagulant, and fibrinolytic factors).The extent of thrombogenicity may determine the progression of atheroma and the clinical manifestation of atherothrombotic events, with the highest thrombogenicity in African Americans and lowest in East Asians. Inherent thrombogenicity may influence clinical efficacy and safety of specific antithrombotic treatments in high-risk patients, which may in part explain the observation that East Asian patients have reduced anti-ischemic benefits and elevated bleeding risk with antithrombotic therapy compared to Caucasian patients. In this review, we discuss available evidence regarding the racial differences inthrombogenicity and its impact on clinical outcomes among patients with atherosclerotic cardiovascular disease.

@#Despite the huge loss of lives and massive disruption of the world economy by the COVID-19 pandemic caused by SARS-CoV-2, scientists are yet to come out with an effective therapeutic against this viral disease. Several vaccines have obtained ‘emergency approval’, but difficulties are being faced in the even distribution of vaccines amongst high- and lowincome countries. On top of it, comorbidities associated with COVID-19 like diabetes, hypertension and malaria can seriously impede the treatment of the main disease, thus increasing the fatality rate. This is more so in the context of sub-Saharan African and south Asian countries. Our objective was to demonstrate that a single plant containing different phytoconstituents may be used for treatment of COVID-19 and comorbidities. Towards initial selection of a plant, existing scientific literature was scanned for reported relevant traditional uses, phytochemicals and pharmacological activities of a number of plants and their phytoconstituents pertaining to treatment of COVID-19 symptoms and comorbidities. Molecular docking studies were then performed with phytochemicals of the selected plant and SARS-CoV-2 components – Mpro, and spike protein receptor binding domain and hACE2 interface using AutoDock Vina. We showed that crude extracts of an indigenous African plant, Costus afer having traditional antidiabetic and antimalarial uses, has phytochemicals with high binding affinities for Mpro, and/or spike protein receptor binding domain and hACE2 interface; the various phytochemicals with predicted high binding energies include aferoside C, dibutyl phthalate, nerolidol, suginal, and ±-terpinene, making them potential therapeutics for COVID-19. The results suggest that crude extracts and phytochemicals of C. afer can function as a treatment modality for COVID-19 and comorbidities like especially diabetes and malaria.

ABSTRACT

@#COVID-19, caused by the SARS-CoV-2 virus, can lead to massive inflammation in the gastrointestinal tract causing severe clinical symptoms. SARS-CoV-2 infects lungs after binding its spike proteins with alveolar angiotensin-converting enzyme 2 (ACE2), and it also triggers inflammation in the gastrointestinal tract. SARS-CoV-2 invades the gastrointestinal tract by interacting with Toll-like receptor-4 (TLR4) that induces the expression of ACE2. The influx of ACE2 facilitates cellular binding of more SARS-CoV-2 and causes massive gastrointestinal inflammation leading to diarrhea. Diarrhea prior to COVID-19 infection or COVID-19-induced diarrhea reportedly ends up in a poor prognosis for the patient. Flavonoids are part of traditional remedies for gastrointestinal disorders. Preclinical studies show that flavonoids can prevent infectious diarrhea. Recent studies show flavonoids can inhibit the multiplication of SARS-CoV-2. In combination with vitamin D, flavonoids possibly activate nuclear factor erythroid-derived-2-related factor 2 that downregulates ACE2 expression in cells. We suggest that flavonoids have the potential to prevent SARS-CoV-2 induced diarrhea.

ABSTRACT

The current research work was to develop bilayer tablet of venlafaxine hydrochloride to increase drug efficacy for efficient treatment of depression. The satisfactory result of treatment can be achieved upon the maintenance of drug concentration within an effective level in the body, so a uniform and constant drug supply are desirable. An immediate layer of venlafaxine HCl was formulated using super disintegrants, i.e., croscarmellose sodium (CCS) and sodium starch glycolate (SSG); tablet compact by direct compression. HPMC K100M and ethylcellulose (EC) were utilized as release retarding polymers in sustained release layer by wet granulation technique with the help of PVP K30 in IPA solution (10%) as a granulating agent. Full 32 factorial designs were used to find out the optimum quantity of release retardant polymers. Bilayer tablet was evaluated for various parameters, i.e. hardness, friability, weight variation, % drug content, disintegration time (IR layer), and % drug release study. Statically, an analysis was carried out using factor X1 (HPMC K100M) and X2 (EC) for dependent variable % drug release at 8, 12, and 20 hours. A formulation was optimized and a formulation containing 305.36 mg of HPMC K100M and 54.03 mg of ethyl cellulose. Optimized formulation show 47.12 ± 2.1, 59.89 ± 2.2, and 89.06 ± 2.3 drug release at 8, 12, and 20 hours, respectively, which is almost similar to theoretical dose calculation with similarity factor f2 97, 99, and 98%, respectively. Bilayer tablet formulation was observed to be stable and fulfilled all compendia specifications.

Background: Childhood Trauma Questionnaire-Short Form (CTQ-SF) is an instrument to assess child abuse and neglect Aim: to adapt and confirm the psychometric properties of the Spanish version of the CTQ-SF in Chile. Material and Methods: The CTQ-SF was applied to 89 clinically depressed subjects (77.5% women) who consulted at an outpatient mental health clinic. Confirmatory factor analysis (CFA), reliability (Cronbach's α) tests, and convergent validity analyses with clinical markers of complex depression were carried out. Results: The Chilean version of the CTQ-SF demonstrated an acceptable fit to a five-factor model, with adequate psychometric properties. The CFA revealed that a better fit to a five-factor model would be achieved after elimination of two items from the physical neglect scale, the less reliable scale of the questionnaire. The physical abuse scale discriminated between patients with a complex depression versus non-complex depression, and all the CTQ-SF's scales discriminated between patients with high suicide risk and/or history of psychiatric admissions versus those patients without this background. Conclusions: the Chilean version of the CTQ-SF shows evidence of structural and discriminant validity, and reliability, in a clinical sample. Better alternatives to specifically assess the physical neglect construct should be developed.

Bachground /aim Coronary artery imaging is one of the most commonly used diagnostic methods. We aimed to investigate whether there is a correlation between left main coronary artery (LMCA), left anterior descending artery (LAD) and left circumflex artery (LCx) artery dimensions in normal cases and a possibility to express the coronary dimensions by multiple linear equations. Materials and methods Images of coronary angiograms of 925 normal cases selected from 3855 cases made up the study population (515 men and 410 women; age range, 30–75 years). The mean age of the patients was 55.50 ± 6.49 years. The mean body mass index was 24.79 ± 1.45 kg/m2 (range, 31.30–21.26 kg/m2). The mean dimensions of LMCA, LAD and LCx were 4.18 ± 0.65 mm, 3.22 ± 0.63 mm and 3.07 ± 0.65 mm, respectively. Correlation between LMCA, LAD and LCx diameters was investigated. Multiple linear regression analysis was used to develop a model to elucidate the relationship between LMCA, LAD and LCx diameters. Results There was a strong correlation between LMCA dimensions and LAD and LCx dimensions (r = 0.526**, p < 0.001* and r = 0.469**, p < 0.001*, respectively). The positive correlation indicated that a regression analysis can be carried out by incorporating the measurements. Coronary artery dimensions were gender specific. Conclusion The present study explored the possibility of explaining the relationship with the LMCA and its branches by multiple linear equations, which may then be used to estimate the reference diameter of a stenosed coronary artery when the other two arteries are normal.

Aim: Effect of gamma irradiation on genomic disorder in sesame are scanty. The present study was undertaken to evaluate the mutagenic effects of gamma rays on different parameters in two popular cultivars of sesame, Roma and Tilottama. Methodology: Seeds of these two cultivars were irradiated with five doses of gamma rays: (250, 300, 350, 400, and 450 Gy) at BARC, Trombay and were sown (along with the un-irradiated control) during March 2015 in a split plot design with 3 replications keeping row to row and plant to plant distance at 30 cm and 10 cm, respectively to determine mutagen sensitivity with regard to pollen fertility (%), germination (%) and seedling height (cm), root-shoot length (cm), plant survival (%) at maturity in M1 generation. To study mutability, four to five capsules from each M1 plants in all the treatments were collected separately to give rise the M2 generation. Individual plant progeny rows were sown in M2 during March 2016. ID50 was determined by probit analysis for germination, shoot-root length and plant survival. Since the dose requirement for pollen fertility is very high, ID30 was calculated instead of ID50 value. Results: It was observed that with increasing doses of gamma rays, the response of all characters decreased significantly and followed a linear relationship in both varieties. The root system was more profound to gamma rays than the shoot. Chlorophyll mutations showed independent response to different doses of gamma rays as they occurred in random. The mutability of genotype Roma induced with different doses of gamma rays was higher than that of Tilottama. Interpretation: Due to saturation in the mutational actions, response of characters decreased with increase in gamma ray doses but magnitudes of effect differed between genotypes. The cultivar Tilottama was found to be more sensitive than Roma.

Background: Adenosine deaminase (ADA) is suggested to modulate the bioactivity of insulin, but its clinical significance in Type 2 diabetes mellitus (DM) is not yet established. The present study was undertaken to evaluate serum ADA activity and serum uric acid levels in patients of Type 2 DM. Aim: To evaluate the serum ADA level and to correlate ADA levels with Blood Glucose, Glycated Hemoglobin (HbA1c) levels in Type-2 DM patients. Material and Method: It is a case control study. The subjects in this study were divided into 3 groups. Group I consisted of 50 normal healthy individuals who served as controls with no history of DM. Group II consisted of 50 patients of Type 2 DM both males & females in the age group of 40-65 years on oral hypoglycemic drugs with HbA1c <7%. Group III consisted of 50 patients of Type 2 DM both males & females in the age group of 40-65 years on oral hypoglycemic drugs with HbA1c >7 %. Serum levels of fasting blood sugar, HbA1c, ADA and uric acid were estimated in all the subjects. Results: FBS, HbA1c, ADA and serum uric acid levels were found to be increased in the patients of Type 2 DM as compared with controls. Conclusion: It is concluded that there is an increase in serum ADA levels with increase in HbA1c levels. Serum uric acid levels increased with moderately increasing levels of HbA1c <7% and then decreased with further increasing levels of HbA1c >7% (a bell-shaped relation).

ABSTRACT

This paper explores the effects of two run-of-river dams (Jirau and Santo Antônio) built in cascade in the middle Madeira River (Brazil) on the interruption of long-distance migration routes of the gilded catfish (Brachyplatystoma rousseauxii). A participative monitoring system was set up to compare capture by commercial fishers approximately 1500 km upstream of the dams in Bolivia, before (1998-2007) and after (2015-2017) dam closure. A significant decrease in gilded catfish catches and in catch per unit effort was observed after dam closure, whereas no significant difference in mean weight was found. Back-estimation of age suggests that the few individuals remaining after dam closure in 2009 are a mixture of old homers that returned upstream before dam closure, and residents hatched after dam closure and trapped in the upper Madeira. Unless fishways in the Madeira River dams improve their efficiency, the gilded catfish might become rare and in danger of regional extinction in the upper Madeira basin in the next few years, negatively affecting river fisheries.(AU)


Este estudio explora el impacto de las represas en cascada Jirau y Santo Antônio en el curso medio del río Madeira (Brasil) sobre la interrupción de la ruta de migración del bagre dorado (Brachyplatystoma rousseauxii). Se estableció un sistema de monitoreo participativo para comparar la captura de dorado por pescadores comerciales en Bolivia, aproximadamente 1 500 km río arriba de las represas, antes (1998-2007) y después (2015-2017) de su cierre. Se observó una disminución significativa en las capturas y en la captura por unidad de esfuerzo de dorado después del cierre de ambas represas. No hubo diferencias significativas en el peso promedio antes y después del cierre. La estimación de la edad de los pocos individuos capturados arriba de las represas sugiere que se trata de una mezcla de individuos adultos que han podido pasar las represas antes de su cierre en 2009, y residentes nacidos después del cierre de las represas, atrapados en el alto Madeira. Si los sistemas de traspaso de peces implementados en las represas del rio Madeira no mejoran su eficiencia, el dorado podría volverse escaso y en peligro de extinción regional en la cuenca alta del Madeira en los próximos años, afectando negativamente la pesca de esta especie.(AU)

BACKGROUND@#The comparative outcomes of subcutaneous implantable cardioverter-defibrillator (S-ICD) and transvenous ICD (T-ICD) have not been well studied. The aim of this study was to evaluate the safety and efficacy of currently available S-ICD and T-ICD.@*METHODS@#The study included 86 patients who received an S-ICD and 1:1 matched to those who received single-chamber T-ICD by gender, age, diagnosis, left ventricular ejection fraction (LVEF), and implant year. The clinical outcomes and implant complications were compared between the two groups.@*RESULTS@#The mean age of the 172 patients was 45 years, and 129 (75%) were male. The most common cardiac condition was hypertrophic cardiomyopathy (HCM, 37.8%). The mean LVEF was 50%. At a mean follow-up of 23 months, the appropriate and inappropriate ICD therapy rate were 1.2% vs. 4.7% (χ = 1.854, P = 0.368) and 9.3% vs. 3.5% (χ = 2.428, P = 0.211) in S-ICD and T-ICD groups respectively. There were no significant differences in device-related major and minor complications between the two groups (7.0% vs. 3.5%, χ = 1.055, P = 0.496). The S-ICD group had higher T-wave oversensing than T-ICD group (9.3% vs. 0%, χ = 8.390, P = 0.007). Sixty-five patients had HCM (32 in S-ICD and 33 in T-ICD). The incidence of major complications was not significantly different between the two groups.@*CONCLUSIONS@#The efficacy of an S-ICD is comparable to that of T-ICD, especially in a dominantly HCM patient population. The S-ICD is associated with fewer major complications demanding reoperation.
